Refund Policy

1. General Provisions
This document defines the refund policy for the Form.com.pl platform operated by AlfaMedial, located at Os. Bohaterów Września 79a/16, 31-621 Kraków, Poland (VAT ID: 6771425568, REGON: 120607210).
The platform serves as a payment intermediary on behalf of partner entities. AlfaMedial does not provide the final service itself but enables online payments and forwards funds to the respective service provider.
2. Cancellation and Refund Conditions
The user may cancel a paid service or booking no later than 7 days before the scheduled date (e.g., reservation, transfer, event).
In such cases, AlfaMedial will issue a full refund to the same bank account or payment card used for the original transaction.
Cancellations made less than 7 days before the scheduled service date are not eligible for a full refund. Any exceptions are handled individually, in consultation with the relevant service provider.
3. Refund Procedure
Refunds are processed only after:
– AlfaMedial receives a written cancellation request from the user,
– verification of the payment in the Paynow system,
– confirmation from the service provider that the service has not been rendered.
Refunds are sent to the same account or card used for the original payment. The refund process takes up to 14 business days after verification and approval.
4. How to Request a Refund
A refund request may be submitted via:
– the contact form at form.com.pl/kontakt,
– by e-mail to: info@form.com.pl.
The refund request should include:
– full name,
– e-mail address used for the payment,
– transaction reference number,
– date and amount of payment,
– reason for requesting a refund.
5. Failed or Incorrect Payments
If a payment fails due to technical issues or system errors, AlfaMedial will verify the transaction within the Paynow system and refund the amount accordingly.
The user will be notified by e-mail about the verification results and the expected refund date.
6. Refunds Can Only Be Made to the Original Account
For security reasons and in accordance with anti-money laundering (AML) and counter-terrorism financing regulations, refunds are processed only to the same account or payment method used for the original payment. Refunds to alternative accounts are not permitted.
7. AlfaMedial’s Responsibility
AlfaMedial is responsible solely for the proper mediation of the refund process and transferring funds to the correct service provider. The company is not liable for the individual cancellation policies or refund decisions made by the service providers themselves.
8. Refund-Related Complaints
Complaints related to refunds may be submitted within 30 days from the date of payment via:
– the contact form on the website,
– e-mail at info@form.com.pl.
All complaints are reviewed within 14 business days of receipt.
9. Final Provisions
This Refund Policy forms an integral part of the Terms of Service of Form.com.pl. AlfaMedial reserves the right to update this policy at any time. The latest version is always available on the website.
